You will need to fill out one registration per child. Once you have completed the registration and payment you can start a new registration form for the next child.
Your child must be 4 years old by 30 August 2022 to register. You will need to provide a birth certificate for your child prior to collecting your uniform.
By submitting the form, I, as the parent/guardian of the registrant, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the registrant's life, limb, or well-being.
By submitting the form, I, the parent/guardian of the registrant, a minor, agree that I and the registrant will abide by the rules of the FA. I, also give consent for any images or content taken at training and games to be used for promotional use for JTL FC Academy.
